Home Delivery Service

Green Forest Library offers Home Delivery Service for patrons approximately five miles or less from the library (exceptions can be made on a case-by-case basis).

Deliveries are generally made on Wednesday afternoons by library staff. Please print and fill out this form and return it to the library to be eligible for home delivery service.

If you do not have a printer, or cannot make it to the library, please call us at 87-438-6700 and we can mail you a copy of this form to fill out.
